I think CDPR didn't realize how popular Keanu would be, and so when they revealed him at E3 they had to panic and rewrite the game
Until recent patches, the game clearly indicated that Keanu was supposed to show up later and was shoehorned into quests. The first quest you're likely to do after being attacked by Silverhand in your apartment is to go get your car, which is crashed into. Until a very recent update, you and Keanu were buddy-buddy during that quest even if he just tried to kill you minutes before. In another instance, a character who you're shown working for during the 6 month timeskip acts as if they've never met you when you enter their territory for the first time
The fact there's about an hour long unskippable, barely interruptible segment between the No-Tell Motel and V getting turned loose on the open world, with a fakeout ending halfway through, really points to the heist being an end of act 2 shocking finale and all the Silverhand stuff would've been a late game act 3

From the wreckage photos it looks like a controlled flight into terrain - same shit that got Kobe, in the same weather circumstances to boot. I suspect the pilot did attempt an emergency landing in the rough weather and while trying to set it down collided with the mountain, hence the hard landing bit
RED at least made the conscious decision to change that a little bit, in that cyberware done for cosmetics, for gender affirmation, for amputation replacement, etc, don't turn your character into a cyberpsycho and don't affect them negatively (gameplay wise)
It's usually only once you start taking on cyberware that makes your character more than human that cyberpsychosis becomes a concern
Which is far better than how Shadowrun handles it, which is that any cyberware begins to negatively affect your character's maximum charisma, humanity, and magic stats, even something as simple as a single replaced limb or a pair of replaced eyes
While simultaneously believing that a single Western tank can destroy an entire brigade of Russian ones without taking damage, and that a single F-16 is sufficient to maintain a permanent no fly zone
