I have mine directly connected to my AVR and have never experienced an audio delay so I can't say if it's fixed.
No reason to pay for a subscription if you are not watching any of the content.

It's fine on a HDMI 2.0 input you will not be missing any features.
You need to upgrade to the 4K plan for the best picture on content that is available in 4K. 1080p content might look slightly better with the Apple TV 4K because it will up scale it to 4K, your TV has poor up scaling , Netflix 1080p streams are low bitrate though, even their 4K streams are lower than other services.
Insignia TVs are not know for their picture quality so the Apple TV will not be able overcome the shortcomings of the TV. SDR will look better than HDR on that model it does not get bright enough to display HDR properly.
That's a 2015 device that only does 1080p. If you have a 1080p TV or TV that does good up scaling it's not too bad but on 4K TV midrange TV the picture will be bad with Netflix. Netflix streams 1080p at very low bitrates.
According to Rtings you need to set Digital audio out to Auto 1 and enable passthrough auto mode.
