It's often (but not always) available at supermarkets, but it's not commonly eaten. Hopefully the tradition will die off with the older generations.

If he doesn't have access to a car, he may be unable to get to a doctor/hospital on his own. Many parts of the US are like this.

That's a big part of it, but some people are just loud and some restaurants just play their music way too loud all the time.

My experience in American restaurants is that the music is usually whatever is currently popular, so there's a lot of hip hop and pop songs about dancing and fucking.

Poorer areas have fewer doctors and they are stretched thin. This is a socioeconomic problem more than a racial one, but the two are intertwined.
