I keep finding things like direct replies to my posts and to my comments in the wild instead of via my inbox because my stuff is marked as read before I’ve interacted with the page. Any idea why this might be?

There's a bug(?), that if someone replies to a reply to you it becomes marked as read/disappears from your notifications.

Yeah it happens to me all the time so I've started compulsively checking comments to see if I'm not just ignoring someone.

Same. It's especially bad if someone includes a link that sets off the mirror bot, because then it instantly gets marked as read without ever appearing in the inbox at all.
