Not quite accurate. His actions are more of an unexpected result, but not the root.
It might be difficult to fathom if you only know the term from Star Trek, but the "ensign" rank is an officer's rank. Ensigns are officers. An ensign is a type of officer.
So Wesley Crusher was an officer?

What's the ratio of ships to personnel? The attrition rate must be phenomenal to be able to guarantee a captaincy after 8 years. That explains the red-shirt phenomenon.
I've had that same thought everything I see that part

Or perhaps most officers tend to burn themselves out and quit Starfleet before then.
