If Chernobyl was anything to go by (i know, I know, communism has never actually been done, yada yada yada)? Cover it up and act like nothing is happening, disappear anyone trying to actually do something, and blame everyone else when the climate collapse starts and ignoring it is no longer possible. In the end, act like you did everything you could with the information you had at the time.

Chernobyl did get solved tho, at great personal sacrifice by some very brave people.
So, basically the same thing that's happening now...
Thing is, no economic system will eliminate people that are afraid of the truth and are obsessed with covering their own asses. Best we can do is shift culture to reward being more up front and honest but it's a pretty tall order to expect someone to happily be humiliated when they make a mistake even if there aren't lives on the line.
