A preliminary report by Indian aviation accident investigators said on Saturday the fatal Air India crash that killed 260 people in Ahmedabad last month showed the plane engines' fuel switches shifting from run to cutoff within seconds of each other.

It seems very unlikely to have been a mechanical failure, but we don't yet know if it was an intentional action or an accident.
No recommendations have been made to Boeing or any pilots, so we can assume it's still safe to fly.

The final report should have more answers, but that could be several years away.
