I have 13 Seagate Exos X22 hard drives on an LSI MegaRAID 9361-16i in RAID 6. They are on ports 0-11 and 13 (skipped 12 for a later addition and 14-15 are also empty). Ports 5, 6, & 7 keep dropping from the virtual disk. These same ports are showing as a "Backend SAS Address" as "0x0", while all other drives/ports are showing a full HEX address. I can only guess that this is related...?

I have replaced all the hardware from the controller to the drive and everything in between. The same PORTS continue to drop from the VD. The controller, cables, drives, backplanes (even tried without the backplanes, drives directly attached to the controller). Swapping the locations of the drives results in the same ports dropping, so I know it's not the drives. SeaTools also reports good drives.

I'm pulling out my hair. Any advise?

Specs:

ASRock Z390 Phantom Gaming 7, 64G RAM, Core i9 9900, 2TB NVMe, Windows 10, 750Watt Corsair PSU

Mellanox 2x 25Gb SFP+ NIC in x16 PCIe slot 1 (running at x8)

x1 PCIe slot 2 empty

x1 PCIe slot 3 empty (have also tried running an expander in this slot with same issue)

LSI (Avago) MegaRAID SAS 9361-16i - x16 PCIe slot 4 (running at x8)

x1 PCIe slot 5 empty

x4 PCIe slot 6 empty
