It really depends on what your definition of "is" is.
The man pictured is former US President Bill Clinton. During his term in office he was accused of having an affair with a White House (name of the presidential offices of the USA) intern named Monica Lewinsky. Specifically, there were accusations of her having performed fellatio on him. The impaction of the picture is that hidden under the desk is Monica Lewinsky with her mouth full of presidential cock.
I think you can go past accusations. The transcripts were released so there are official records where he admitted to the acts, and she has always been up front about it, once it came out.
