There are probably many cases where they have to debride the wound first for this to be effective (Pseudomonads like to form biofilms). Very painful.

Hospitals in Gaza are resorting to performing surgery without anaesthetic currently.

If the idea of debridement gives you the ick then spare a thought for the people who are having to undergo this without any pain relief.
