I was thinking more of a "I have no mouth, and I must scream" kind of scenario where no matter how much he wants to die we keep him alive to suffer more and more humiliation and grovelling.

Don't feed him McDonalds like he wants, make it a veggie Burger King Whopper liquified in a slurry with a full sugar coke once a year to celebrate his removal from office.
