The article answers the questions pretty well:
Mental illnesses don't go in your standard file. You have to give permission. If you don't give permission, the people at the draft office can't see your diagnosed illness.
If you then tell them... Well, they can't tell the difference between someone telling the truth and making shit up to dodge the draft.
